What Conditions Are Eligible for Medical Lien Treatment?

TL;DR: Medical lien care is appropriate for injuries from an accident or negligent event when a legal claim is pending and treatment is medically necessary. We treat acute and chronic pain from motor-vehicle crashes, falls, and other personal-injury cases—coordinating closely with your attorney while you focus on recovery.

When a medical lien makes sense

You may qualify for treatment on a lien when all of the following are true:

  • Your injury is related to a personal injury event (e.g., auto collision, slip/trip and fall, assault, dog bite, defective product).
  • You have an active claim and (usually) an attorney who can sign our lien agreement.
  • Recommended care is medically necessary and tied to the incident (documented causation).
  • Traditional coverage (health insurance/MedPay) is unavailable, exhausted, or not promptly accessible.

Note: Workers’ compensation usually follows different rules and is billed to the WC carrier rather than a PI lien.

Common conditions we accept on lien

Spine & nerve

  • Whiplash/cervical strain–sprain
  • Lumbar and thoracic strain–sprain
  • Cervical or lumbar radiculopathy/sciatica
  • Disc bulge or herniation (confirmed or suspected)
  • Facet and sacroiliac (SI) joint pain

Upper extremity

  • Shoulder: rotator cuff strain/tear, impingement, labral injury
  • Elbow/wrist/hand: sprains, tendonitis, nerve entrapment (e.g., carpal tunnel exacerbation)

Lower extremity

  • Knee: MCL/LCL sprain, meniscal injury, patellofemoral pain
  • Ankle/foot: sprain/strain, plantar fasciitis post-trauma, contusions

Head & other

  • Concussion/mild traumatic brain injury (mTBI) symptoms (headache, dizziness, light/noise sensitivity)
  • Headaches, jaw/TMJ pain after impact
  • Myofascial pain and trigger points
  • Rib/chest wall contusions, thoracic outlet–type symptoms after restraint forces
  • Exacerbation of prior stable conditions clearly aggravated by the event

We assess for red flags (fracture, infection, progressive neurological deficit) and direct urgent care if needed.

What we provide under a lien

  • Comprehensive evaluation & documentation (history, exam, causation analysis)
  • Evidence-based care plan (home program, PT/OT referrals, medications when appropriate)
  • Interventional options when indicated (e.g., trigger point, joint, or spine-related procedures via referral)
  • Imaging coordination (X-ray/MRI/ultrasound) when results will change care
  • Regular progress reports to your attorney and case manager
  • Clear impairment/work status notes when applicable

What we typically cannot accept on lien

  • Conditions unrelated to the incident (purely degenerative findings without aggravation)
  • Elective or non–medically necessary services
  • Long-term opioid management unrelated to acute injury
  • Cases without a signed lien agreement from an attorney or responsible party

What we need to get started

From the patient:

  • Photo ID, incident date, a brief description of what happened
  • Prior relevant records (if available)

From the attorney/representative:

  • Signed medical lien agreement (our standard form or yours)
  • Claim details (adjuster, claim number, policy limits if known)
  • Prior imaging and treatment records, if available
  • MedPay information (if applicable)

We’ll take it from there—authorization coordination, scheduling, and ongoing updates.

FAQs

Do I need an attorney to be seen on a lien?
In nearly all PI lien cases, yes. An attorney signs the lien and receives our reports.

What if I also have health insurance or MedPay?
We’ll align billing with your case strategy. Some services may route through insurance/MedPay first; others may remain on lien depending on the plan discussed with your attorney.

Will I have out-of-pocket costs during treatment?
Typically, payment is deferred until case resolution under the lien terms. Any exceptions will be discussed up front.

How soon can I be seen?
Once the lien is in place, we prioritize scheduling and initial evaluation to prevent delays in care and documentation.
